Welcome, and thank you for joining the Leila Day community!
After you submit a signed copy of your contract with a deposit, you are considered an enrolled family. To prepare for the upcoming school year, we will reach out to you occasionally with information and instructions about completing enrollment forms, what to expect over the summer and on your first days of school, and other additional forms that may be required depending on your child’s individual health history and needs. Below is a general outline of what you can expect:
– A link to your child’s enrollment forms will be emailed to you in May with a due date in June
– Health Forms are due August 15th
– Home Visits for students new to Leila Day are in August

– If your child is in the After School Program
Health forms are due by August 15th for the upcoming school year. It is required by the State that we have your child’s current health examination record* as well as any additional health forms allowing Leila Day staff to possess and administer any medications your child may need while in our care. We are unable to allow a student to attend if there are missing/incomplete forms and/or medication.

Be sure to download and print or request a copy of our forms to take with you to avoid repeated trips to your pediatrician.
Health Record Instructions
o *Current = examination date within the past 12 months
o Be sure to complete the entire side designated for parents.
o Be sure your health care provider completes the new square on the form for TB screening.
o If you cannot have a current Health Record to us as requested, the Connecticut State Department of Health Licensing Regulations requires that you provide verification of a scheduled visit from your health care provider. This is due by August 15th.
o Your child’s enrollment in school will be delayed if you have not submitted:
o a current Health Record or verification of a scheduled appointment
o your child’s medication along with the completed Authorization to Administer Medication form.
In order to ease your child’s transition to Leila Day, teachers make a brief home visit to meet children in their own familiar home environment. If this is your child’s first year at Leila Day, one of the teachers in your child’s class will call you prior to the start of school to arrange the home visit.
After School Program
If your child is in the After School Program, please be aware that our first day does not always line up with New Haven Public School. Check the calendar or contact us with any questions. If your child is new to the Leila Day After School Program, we recommend that you meet your child at Worthington Hooker School and walk back with us on that day. Also to ease your child’s transition into the program, it can be helpful to arrange for early pick up from Leila Day for the first few days.
